Trying to Conceive After 35? Here Is What You Actually Need to Know.

If you are 35 or older and trying to get pregnant, you have probably been told that the clock is ticking. That phrase, while technically grounded in biology, is also one of the most unhelpful things a woman in this situation can hear. Yes, fertility changes with age. But the story does not end at 35, and the options available to you now are genuinely good.

What Actually Changes After 35

The main change is egg quantity and quality. Women are born with all the eggs they will ever have, and that supply declines over time. More significantly, the chromosomal quality of eggs decreases with age, which means a higher proportion of embryos may be chromosomally abnormal and unable to result in a healthy pregnancy. This is why conception takes longer naturally after 35, and why the chance of miscarriage is somewhat higher. It is not a wall. It is a gradient.

How Soon Should You Seek Help?

The medical guideline for women over 35 is six months of trying before seeking a fertility consultation, rather than the standard twelve months for younger women. This is not alarmism. It is a practical acknowledgment that time is a meaningful factor and that earlier investigation gives more options. If you are 38 or older, we would generally recommend evaluation without delay at all, because the difference between starting treatment now versus in six months can be significant.

What IVF Can Achieve After 35

IVF success rates do decline with age, and we believe in being honest about that. For women between 35 and 37, success rates at a good centre remain in the range of 35 to 50 percent per cycle. Between 38 and 40, that figure typically falls to around 25 to 35 percent. After 40, natural egg IVF success rates fall further, though many women still achieve successful pregnancies. The key variables are ovarian reserve (measured by AMH and antral follicle count), the quality of the embryos produced, and the experience of the clinical team.

PGT-A: Selecting the Best Embryos

One option that becomes increasingly relevant for women over 35 is Preimplantation Genetic Testing (PGT-A), which screens embryos for chromosomal abnormalities before transfer. By transferring only chromosomally normal embryos, we can reduce the risk of failed implantation and miscarriage significantly. It is not suitable for every situation, but for women over 38 particularly, it is worth discussing during your consultation.

The Role of Egg Freezing for Younger Women

For women in their late 20s or early 30s who know they want children but are not yet in a position to have them, egg freezing is worth considering. It is essentially a form of insurance, preserving eggs at their current quality for future use. Egg freezing is not a guarantee, but for women under 35 it provides a meaningful safety net that is not available once that window has passed.

When Donor Eggs Become the Conversation

For women over 42 or 43, or those with very low ovarian reserve at any age, donor eggs often represent the clearest path to pregnancy. Using eggs from a younger donor dramatically improves success rates, and the resulting child is genetically the partner’s and carried and delivered by the mother.
